Iverjohn-6 may be prescribed when ivermectin is appropriate for a susceptible parasitic infection.

Intestinal strongyloidiasis: treats intestinal infection caused by Strongyloides stercoralis.

Onchocerciasis: treats infection caused by Onchocerca volvulus.

Scabies — Off-label use: oral ivermectin is not FDA-approved for scabies, although CDC guidance includes oral ivermectin as an option for appropriate patients.

Other uses should be based on a clinician's assessment and applicable local guidance.

Ivermectin is not approved or authorized for prevention or treatment of COVID-19.

Ivermectin dosage is weight-based, so the correct number of 6 mg tablets must be determined from the diagnosis and patient's weight.

Do not independently calculate, increase, repeat or combine doses. Follow the regimen prescribed for the diagnosed infection.

For the FDA-labeled human indications:

Take ivermectin orally exactly as prescribed.

Take the tablets on an empty stomach with water for the labeled strongyloidiasis and onchocerciasis regimens.

Use the number of 6 mg tablets specified for the patient's body weight and condition.

For strongyloidiasis, follow-up stool examinations may be needed to confirm eradication.

For onchocerciasis, follow-up and possible retreatment may be required because ivermectin does not kill adult Onchocerca parasites.

Administration can differ for off-label conditions. For example, CDC scabies guidance recommends oral ivermectin with food to increase bioavailability.
Possible adverse effects include:

Dizziness, nausea, vomiting or diarrhea

Fatigue, weakness, headache or abdominal discomfort

Itching or rash

Fever or joint symptoms, particularly inflammatory reactions associated with treatment of onchocerciasis

Rare but serious neurologic reactions, including confusion, impaired coordination, seizures or loss of consciousness

A clinically important interaction is warfarin: post-marketing reports describe increased INR when ivermectin is co-administered with warfarin. Patients taking anticoagulants should tell their prescriber before treatment.
Important Patient Considerations

Iverjohn-6 requires particular caution in several situations:

Children under 15 kg: safety and effectiveness have not been established.

Pregnancy: adequate controlled studies are lacking, and ivermectin should not be used during pregnancy unless specifically determined appropriate by a qualified clinician.

Breastfeeding: ivermectin is excreted into human milk; treatment should be based on an assessment of maternal benefit and potential infant risk.

Loa loa exposure: people with significant exposure in endemic areas of West or Central Africa may require assessment for loiasis before ivermectin because severe neurologic reactions can rarely occur.

Immunocompromised patients: strongyloidiasis can be difficult to eradicate and may require specialist follow-up and repeated treatment.
Iverjohn-6 is an oral human ivermectin medicine and should be used only for an appropriate medical indication. Do not substitute veterinary ivermectin products for human ivermectin tablets. Animal formulations can have different concentrations and ingredients, and the FDA warns that using animal ivermectin in people can cause serious harm.

Taking excessive ivermectin can cause nausea, vomiting, diarrhea, low blood pressure, dizziness, loss of coordination, seizures, coma and potentially death. Do not take extra tablets because symptoms persist or because a previous treatment was ineffective.

Tell the prescriber about pregnancy or breastfeeding, body weight under 15 kg, liver or other significant health problems, current medicines and any relevant travel or residence in Loa loa-endemic areas. Warfarin users may require closer INR monitoring.

Seek urgent medical attention for severe dizziness, confusion, difficulty walking, seizures, loss of consciousness, severe allergic symptoms or breathing difficulty.

Ivermectin should not be used for COVID-19 prevention or treatment because it has not been FDA-approved or authorized for that purpose.
