Ivermectin should be used for a confirmed or clinically appropriate parasitic infection rather than as a general-purpose dewormer.

• Intestinal strongyloidiasis: an established oral ivermectin indication caused by Strongyloides stercoralis.

• Onchocerciasis: treats the microfilarial stage of Onchocerca volvulus; follow-up treatment may be required.

• Scabies — Off-label use: CDC includes oral ivermectin as a treatment option, although it is not FDA-approved specifically for scabies.

• Other parasitic conditions: may sometimes be considered by a clinician according to evidence, local guidance and the individual diagnosis.

The correct ivermectin regimen varies by condition. A 40 mg tablet should not be treated as a standard single dose simply because of its strength

Ivermectin dosing is generally weight- and condition-dependent. The current U.S. human tablet label uses approximately 200 micrograms/kg for intestinal strongyloidiasis and approximately 150 micrograms/kg for onchocerciasis, with specific dosing based on body weight.

Those labeled regimens are based on 3 mg tablets, not a 40 mg tablet. Therefore, the 40 mg strength should only be used when a qualified prescriber has specifically selected it and determined the appropriate amount.

For scabies, CDC guidance describes oral ivermectin as an off-label option using weight-based dosing, generally in two doses separated by 7–14 days. The regimen should be selected by a healthcare professional.

The existence of other Covilife strengths does not mean they are automatically interchangeable. Ivermectin doses are selected according to the infection and patient characteristics rather than by choosing the nearest tablet strength.

• The U.S. ivermectin tablet label instructs patients to take the medicine on an empty stomach with water for its labeled strongyloidiasis and onchocerciasis indications.

• Do not repeat treatment because symptoms persist without medical advice.

• Follow any required laboratory or clinical follow-up, particularly after treatment for strongyloidiasis.

Scabies treatment is different: CDC guidance recommends taking oral ivermectin with food when it is selected for scabies because food increases ivermectin bioavailability. This illustrates why the administration instructions depend on the condition being treated.

Possible adverse effects include:

• Nausea or diarrhoea

• Dizziness or weakness

• Headache

• Itching or skin rash

• Swelling or other inflammatory reactions, particularly in some patients treated for onchocerciasis

Ivermectin can rarely cause serious neurological or systemic toxicity, especially when excessive doses are taken. Warfarin is an important interaction consideration because increased INR has been reported after co-administration.

Patients with possible Loa loa infection who have significant exposure to endemic areas of West or Central Africa require particular medical assessment before ivermectin because severe neurological reactions have rarely occurred after treatment.

Covilife 40mg should not be self-dosed simply because it is marketed as a high-strength antiparasitic tablet. The U.S. human ivermectin label is based on lower-strength 3 mg tablets and weight-based dosing; taking a 40 mg tablet without an appropriate prescription can result in excessive exposure. Large ivermectin doses can cause nausea, vomiting, low blood pressure, dizziness, loss of coordination, seizures, coma and other serious toxicity.

Safety has not been established for children weighing less than 15 kg or during pregnancy for several ivermectin uses. Discuss breastfeeding with a healthcare professional before treatment. Patients taking warfarin or other medicines should provide a complete medication list before receiving ivermectin.

For onchocerciasis, people with significant exposure to Loa loa areas require special evaluation because ivermectin can rarely trigger severe encephalopathy in heavily infected individuals. Follow-up may also be necessary because ivermectin does not kill adult Onchocerca worms.

Oral ivermectin for scabies is off-label in the United States; CDC recommends condition-specific treatment rather than treating scabies with an arbitrary high-strength tablet. Ivermectin has not been approved or authorized for prevention or treatment of COVID-19. Animal ivermectin products must not be substituted for human medicines.
