Iverjohn-3

Iverjohn-3 is used when ivermectin is appropriate for a susceptible parasitic infection. Established oral ivermectin indications include:

• Intestinal strongyloidiasis caused by Strongyloides stercoralis.

• Onchocerciasis (river blindness) caused by Onchocerca volvulus.

• Scabies — Off-label use: oral ivermectin is not FDA-approved for scabies, although CDC guidance includes it as a treatment option in appropriate patients.

• Other uses should be considered only when supported by appropriate clinical guidance and local treatment recommendations.

Ivermectin does not treat viral infections such as COVID-19; the FDA has not approved or authorized it for COVID-19 treatment or prevention.

Ivermectin dosing is weight-based and condition-specific. Do not choose the number of Iverjohn tablets yourself.

• The prescriber identifies the parasite and confirms that ivermectin is appropriate.

• The patient's body weight is used to determine the required ivermectin dose.

• The 3 mg tablets are then used according to the prescribed regimen.

• For FDA-labeled strongyloidiasis treatment, the usual reference dose is approximately 200 micrograms/kg as a single oral dose.

• For onchocerciasis, the labeled reference dose is approximately 150 micrograms/kg as a single oral dose.

The 3 mg tablet should not be treated as a standard daily dose. Follow-up testing may be required for strongyloidiasis, and onchocerciasis may require medical reassessment and repeat treatment.

Take the tablets exactly as directed. Do not change the number of tablets, repeat treatment, or use ivermectin for a different condition without medical advice. The administration instructions can differ for off-label conditions; for example, CDC scabies guidance recommends taking oral ivermectin with food to increase bioavailability.

Commonly reported effects can include:

• Dizziness, nausea, vomiting or diarrhea

• Fatigue, abdominal discomfort or headache

• Itching or rash

• With onchocerciasis treatment, inflammatory reactions related to dying microfilariae can include itching, fever, swollen lymph nodes, edema and joint symptoms.

• Ivermectin has rarely been associated with increased INR when taken with warfarin, so anticoagulation monitoring may be necessary.

Severe dizziness, confusion, difficulty walking, seizures, loss of consciousness, severe allergic symptoms or other unusual reactions require prompt medical attention.

Iverjohn 3 mg is not suitable for everyone. Particular care is needed for people who:

• Weigh less than 15 kg, because safety and effectiveness have not been established in this group.

• Are pregnant or planning pregnancy; human pregnancy safety has not been adequately established.

• Are breastfeeding, because ivermectin passes into breast milk in low concentrations.

• Have significant exposure to Loa loa in West or Central Africa, where serious neurologic reactions can rarely occur after ivermectin treatment.

• Take warfarin or other medicines that may interact with ivermectin.

Tell the prescriber about existing medical conditions and all medicines or supplements being used before treatment.

Iverjohn-3 is a prescription ivermectin tablet and should be used for a confirmed or appropriately suspected parasitic infection under professional guidance. Do not take ivermectin simply because symptoms resemble a parasite infection, and do not use veterinary ivermectin products as substitutes for human medicines.

Ivermectin is contraindicated in people with hypersensitivity to ivermectin or an ingredient in the formulation. Important adverse effects can include dizziness, gastrointestinal symptoms, rash and, rarely, serious neurologic reactions such as confusion, altered consciousness or seizures. Patients being treated for onchocerciasis can experience inflammatory reactions as parasites die; people with significant exposure to Loa loa endemic areas require particular assessment.

Pregnancy requires medical evaluation because adequate controlled safety data are lacking. Safety in children weighing under 15 kg has not been established. Ivermectin can also interact with warfarin and may increase INR.

Do not use Iverjohn for COVID-19 prevention or treatment. The FDA has not approved or authorized ivermectin for this purpose. Seek urgent medical help for severe allergic reactions, confusion, seizures, difficulty walking, fainting or significant breathing problems.
