Azithromycin is prescribed for particular bacterial infections rather than every type of infection. Established tablet indications vary by country and product label.

• Respiratory infections, including selected community-acquired pneumonia and acute bacterial sinusitis.

• Throat infections, including streptococcal pharyngitis/tonsillitis when azithromycin is an appropriate alternative.

• Skin and skin-structure infections caused by susceptible bacteria.

Azithromycin belongs to the macrolide class of antibiotics. It acts primarily by binding to the bacterial 50S ribosomal subunit, interfering with bacterial protein synthesis. This prevents susceptible bacteria from producing proteins needed for growth and replication.

Azithromycin remains in tissues for a relatively long period, which allows some infections to be treated with once-daily regimens and relatively short courses. The regimen still needs to match the specific infection and prescribing guidance; a 500mg tablet should not be assumed to be the correct dose for every condition.

There is no single universal 500mg regimen. Current prescribing information gives different schedules according to the infection.

• Take Azax exactly as prescribed and keep doses at the recommended interval.

• Azithromycin tablets can generally be taken with or without food according to current U.S. and UK tablet labeling.

• Swallow the tablet with water; follow the product leaflet if it provides different administration instructions.

• Complete the prescribed antibiotic course unless your doctor tells you to stop or change it.

• Aluminum- or magnesium-containing antacids can affect azithromycin absorption; separate their use according to professional advice and the product information.

Do not double the next dose if one is missed. Follow the missed-dose instructions supplied with your prescription.

Common adverse effects include nausea, diarrhea, vomiting and abdominal discomfort. Changes in taste can also occur. These effects are usually temporary, but persistent or troublesome symptoms should be discussed with a healthcare professional.

Important interaction or risk considerations include:

• QT-prolonging medicines and certain antiarrhythmics can increase the risk of abnormal heart rhythms.

• Aluminum or magnesium antacids may interfere with absorption when taken too closely together.

• Other medicines, including some anticoagulants and medicines affecting cardiac rhythm, should be reviewed by a pharmacist or prescriber.

• Patients with significant liver disease, previous azithromycin-related liver problems or certain heart-rhythm disorders may require an alternative or additional monitoring.

Do not use azithromycin if you have had a serious allergic reaction to azithromycin, erythromycin or another macrolide/ketolide antibiotic. Tell your doctor about previous liver problems associated with azithromycin, heart-rhythm abnormalities, prolonged QT interval, slow heartbeat, heart failure or low potassium or magnesium levels.

Azithromycin can rarely cause serious cardiac rhythm disturbances, particularly in people with existing QT prolongation or other risk factors. It can also cause significant liver injury; seek medical attention for yellowing of the skin or eyes, dark urine or other symptoms suggestive of liver problems. Severe or persistent diarrhea after antibiotic treatment may indicate C. difficile-associated diarrhea and requires medical assessment.

Pregnancy and breastfeeding should be discussed with a healthcare professional. Children may require weight-based dosing and an alternative formulation. Older adults and people taking several medicines should have their medication list reviewed for interactions. Do not use Azax for a viral cold or flu, and do not share leftover antibiotics.
